Athletic Forms

Please note that athletic / concussion forms are required for all student athletes.  They must be completed and submitted prior to tryouts or practices for a sport.  All student athletes and their parents must be educated on concussions once a year.   At the completion of the course you will receive a receipt that must be submitted along with the Concussion Education Form. 

Athletic Forms

In addition to completing and submitting these athletic forms, all students must have a current physical
examination signed by a physician on file with the Marian Health Office at the beginning of each sports
season.  The physical exam must have occurred with the past calendar year.  If a student's exam will
expire during a playing season then he or she will NOT be able to play without an updated exam.
 
Athletic Fees 2012-2013:
Football - $125
Hockey - $150
All Other Sports - $100
