In addition to completing and submitting these forms, all students must have a current physical examination signed by a physician on file with the Marian Health Office at the beginning of each sports season. The physical exam must have occurred with the past calendar year. If a student's exam will expire during a playing season then he or she will NOT be able to play without an updated exam.
